Welcome to your ClearChain Health plan! Our entire team is committed to helping you navigate the onboarding process and provide ongoing support whenever you need it.
Employers choose to offer ClearChain Health plans because of our proven approach to delivering better benefit levels at reduced costs. Our Member Support team is available to help you with any questions or needs you may have. We provide answers to questions about benefits and provider access, verify eligibility, check claims status, and give you access to the tools you need.
Getting started with your ClearChain Health Plan is easy.
Step 1: Access and set up your member portal.
Your ClearChain Member Portal experience provides live, real-time access to your claims, eligibility, and benefits. Dependents will not receive cards but are able to utilize the member information located on the front of the card.
Through your portal, you can easily check the status of your claims, review deductibles and eligibility, request an insurance card, nominate a provider, and read frequently asked questions. Member Portal Step-by-Step Guide, please click here.
Step 2: Nominate your provider
If you haven’t already done so during the initial plan onboarding process through your employer, you will need to nominate your provider to participate in the ClearChain Health program. Simply click the ‘Nominate Your Provider‘ button, and complete the nomination form. Our Member Support team will contact your provider directly to confirm acknowledgment of participation and answer any questions they may have.
Step 3: Contact our Member Support team
We are here to answer your questions, troubleshoot, and pave the way to a smooth plan transition. Sometimes, when you transition to a new healthcare program, there can be a bit of confusion. If you happen to find yourself in this situation, we’re here to help! Use the number on your card to give us a call. We will work with you and your provider to answer questions.

For first-time users:
Log onto www.clearchainhealth.com. Click on the blue Register button, follows the steps to register, and you will have access to your new member account. After registering with your social security number, you can create your new username and password and start exploring ClearChain Health’s real-time Member Portal.

Benefits of Establishing a Direct Relationship Between You, Your Medical Providers, and Your Health Insurance Plan for Members
- You can select your own doctors, including primary care providers (PCPs) or specialists.
- You do not have out-of-network penalties or restrictions because there is no network.
- You do not need a referral to see a specialist.
- Regardless of the care source, all claims are processed and paid on an in-network basis, reducing your financial responsibility.
Your provider is not “in-network”. Just nominate them to participate!
As patients, we are conditioned to ask, “Is my doctor in-network?” when referring to an insurance plan. ClearChain Health is a network-free, open-access program, which means we don’t restrict your provider access with network limitations.
Instead of searching a portal for a doctor that is in-network, near you, with specific credentials, or has specific services, you simply nominate your current provider to participate with ClearChain Health. This means you get to keep the current provider you know and are comfortable working with for your health care needs.
95% of providers and facilities will participate in the ClearChain Benefits program. The nomination process is what confirms your provider’s participation with ClearChain Health, so they agree to submit your claims to us and receive payment from us.
Why do I have to nominate my provider?
Because we do not have a traditional “network” of providers that you are required to choose, we have developed the provider nomination process to connect him or her to your benefits plan. This nomination process is what establishes the provider relationship with ClearChain Health so they agree to submit your claims to us and receive payment from us.
Providers may be unclear about how ClearChain Health works. Just because your provider is not familiar with or in-network with ClearChain Health, does not mean they will not participate.
Since your plan is an open-access plan, it is not associated with a traditional “network” or insurance carrier. This means that some providers don’t completely understand how an open-access program works.
Once you select your preferred provider, our team then contacts that provider facility and coordinates their participation with ClearChain Health. This means you can visit your provider, they will submit claims to ClearChain Health and accept payment.
Looking for a New Provider? 
Sometimes, when employees go through the transition to a new health insurance program, they see it as an opportunity to visit a new provider. With traditional health insurance programs, members must search their insurance directory to find a provider who is in-network.
At ClearChain Health, we empower you with the freedom to find, select and visit your own provider at any time for any reason. However, if you would like assistance with finding a new provider, just call our Member Support team and we will ask you a series of questions to identify your needs.
Then, we will do some research before recommending a few different provider options. If you select one of the producers on the list, our team then contacts that provider or facility and coordinates their acknowledgment of participation in the ClearChain Health program.
